The vision to create the ADRC of Nevada County began over 15 years ago through the Long Term Care Implementation Council (LTC). In 2010, the LTC applied for the State Independent Living Council (SILC) ADRC grant. That same year, the LTC was renamed the Community Living Implementation Council (CLiC), which has served as the ADRC Advisory Committee since the creation of the ADRC of Nevada County.
The ADRC of Nevada County received the first rural ADRC designation in California.
Mission
The ADRC of Nevada County will provide easy access to a broad array of services, a continuum of help in accessing services and advocacy for individuals desiring long term support services and information.
Services in Nevada County
Person-Centered Counseling: Partner organizations have had staff trained in a person-centered model of service to assist individual to plan for Long Term Supportive Services (LTSS) needs.
Care Transition Intervention: A collaboration between Sierra Nevada Memorial Hospital, FREED, Western Sierra Medical Clinic and Community Recovery Resources to decrease hospital re-admissions.
